Supported Living Services:

• Child Foster Care (CFC)
• Adult Foster Care (AFC)
• Acquire needed/desire living situation
• Providing a safe and healthy environment
• Develop and apply decision making/problem solving skills
• Community integration and safety
• Financial, household management
• Supports consumer control and choices
• Provide supervision as specified by the ISP and/or contractual agreements
• Develop plan to assist person to overcome barriers to achieving outcomes
• Respond to life changes, which may affect choice of outcomes

Customized Living Services:

• 24 hour care with 8 hours of nursing services per day
• Consumers ranges between the ages of 30 to 55
• Consumers diagnosed with TBI.
• Individualized care plan detailing the services he or she will receive.
